    Monitoring and Evaluation Officer

    Job Description

    • The M&E Officer will supervise regular data collection across implementing entities and ensure quality of the data by scheduled verifications and validations.
    • S/he will participate in the implementation of M&E field activities while ensuring that Project M&E processes are adhered to in a consistent manner.
    • The person will be responsible for generating and providing the inputs, information and statistics for monthly, quarterly, semi-annual, annual and other reports.
    • S/he will assist and support the use and deployment of program Monitoring and Evaluation tools across all service delivery points.
    • Constant performance and gap analysis will be carried out weekly and monthly by the Officer who will promptly provide feedback on observations to other members of the team.

    Responsibilities

    • Ensure effective monitoring and evaluation of project quality, using donor monitoring tools appropriate audit results, core values and policies, operational plans, and strategic documents
    • Oversee implementation of M&E systems (data reporting templates) through regular field monitoring visits and tracking of Key Performance Indicators as approved by the donor.
    • Supports quarterly program review meetings to facilitate dialogue and learning on project within the OSS using data generated from the program.
    • Conduct data quality assurance within the facility and at the community level.
    • Support management and accountability through routine, accurate collection and reporting of information that confirms status of project activities based on work plans.
    • Develop the quality control systems and mechanisms needed to guarantee proper management of information and data
    • Work with the clinician and program officer or assistant to ensure the accurate collection of prevention and treatment and work to improve the quality of data generated from the facility and community or field.
    • Ensure that all data from the field is entered into the designated databases; Ensure that the data generated is used to support evidence-informed programming;
    • Work with the State Team Lead to conduct the Project assessment.
    • Supervise and coordinate the collection of accurate monitoring reports from the health facilities (monthly, quarterly, semi-annual and annually) on all required indicators
    • Support the development and refinement of Monitoring and Evaluation materials
    • Build the data culture at health facilities to improve data demand and data use
    • Manage and provide training for data analysis at the service delivery outlets and coordinate the technical assistance to team members in the analysis of data to address gaps in program and services
    • Support information technology infrastructure and management for data management tools including patient biometric capture in collaboration with the program officer/assistant.
    • Ensure all databases are properly secured and routinely backed up.
    • Support on-going documentation systems that demonstrate progress towards quality strategic information management for improved ART delivery at the facility and community level.
    • Mentor facility staff to ensure that the data entered into hospital database is accurate, consistent and according to prescribed format through routine visits.
    • Ensures accurate upload of required data on the EMR, NDR, and any other electronic platform used on the program.
    • Ensures proper documentation of the HTS registers, Enrolment registers, ART registers and other electronic and hard-copy management information system tools used in the project.
    • Collates and report weekly, monthly, quarterly, semi-annual and annual program reports.
    • UndertakeotheractivitiesasassignedbytheClinician and or state team lead.

    Program Assistant and Outreach Coordinator

    Job Description

    • The Program assistant & outreach coordinator will provide effective monitoring of the program objectives and its implementation. Ensures close monitoring of OSS target in all program areas and work with facility and community or field team to achieve set targets.

    To support Diadem Consults Initiative Ltd/GTE. achieve the following:

    • S/he will work with the OSS clinician to support the implementation of high-quality care and support activities for HIV Key populations at the facility and community level.
    • The incumbent will support the implementation of KP program activities in line with PEPFAR guidelines within the OSS.
    • Ensures timeous submission of monthly programmatic report.
    • Identify programmatic areas that are lagging behind on service coverage, initiate dialogue and required actions
    • Manages program activities of the community facilitators, peer educators, counselor testers, and community mobilizers with support from the clinician in achieving set program targets.
    • Work with the community facilitators, peer educators, counselor testers, and community mobilizers for active case-finding and demanding creation for KP prevention, HTS, HTS_POS, linkage, retention, viral load coverage and suppression, patient biometric capture, cryotherapy, gender-based violence, PrEP, social networking strategy, Index testing, HIV self-testing and KP adolescent programming.
    • Leads and provide support and guidance for program planning at the facility and community or field level, and managing program development through the implementation stages.
    • Contributes to ongoing strategic communication; solicits support and feedback from clinician and/or State Team Lead as needed.
    • In alignment with strategic direction advises the facility staff on ways to improve programmatic efficiency and effectiveness.
    • Provides regularly scheduled progress reports on program development initiatives.
    • Proactively seek and outsource skilled community facilitators, peer educators, counselor testers, community mobilizers to support project implementation and other program deliverables.
    • Also, the focal person for HIVST, SNS and ICT/PNS in achieving assigned program target including proper electronic and hard-copy documentations and reporting.
    • Participate in the OSS target performance review process, in conjunction with the clinician, and/or state team lead.
    • Maintains regular communication with all field staff and organizes outreaches aimed at meeting program deliverables.
