Resident Doctor / Registrar (Ear, Nose & Throat) at Nnamdi Azikiwe University Teaching Hospital

• Two (2) copies of curriculum vitae
• Two (2) copies each of all relevant credentials Evidence of change of name where necessary Birth certificate/Age declaration
• Three referee reports.
