    Social Worker - Adherence

    Job Description

    • To provide psychosocial support, counseling, and adherence education to clients enrolled in HIV care and treatment.
    • The Social Worker/Adherence Counselor will work closely with the clinical and community health teams to promote positive health outcomes, retention in care, and treatment adherence.

    Key Responsibilities

    • Conduct individual and group counseling sessions for ART clients.
    • Provide psychosocial assessments and develop care plans for clients in need.
    • Support newly diagnosed clients with HIV disclosure, partner notification, and treatment readiness.
    • Educate clients on ART adherence, side effects, and healthy living with HIV.
    • Track and monitor patient adherence using pill counts, appointment keeping, and viral load results.
    • Identify and address barriers to adherence and retention in care.
    • Collaborate with healthcare providers to follow up on defaulters and lost-to-follow-up clients.
    • Identify clients in need of social support and refer them to relevant community-based services.
    • Maintain updated directories of support networks and social welfare services.
    • Facilitate access to services such as nutritional support, transportation, and legal aid where needed.
    • Maintain accurate and confidential records of counseling sessions and client interventions.
    • Assist with reporting adherence and psychosocial indicators to the monitoring and evaluation team.
    • Participate in quality improvement initiatives and case review meetings.
    • Support training and mentoring of peer educators, community health workers, and support groups.
    • Train and distribute IEC (Information, Education, and Communication) materials on adherence and psychosocial support.

    Qualifications and Experience

    • Degree or Diploma in Social Work, Psychology, Counseling, or a related field.
    • Certification or formal training in HIV counseling and testing (HTS).
    • Knowledge of ART guidelines, psychosocial issues affecting PLHIV, and patient confidentiality.
    • Strong interpersonal, communication, and case management skills.

    Nurse Case Manager

    Responsibilities

    • To provide technical support and capacity strengthening in HIV counseling and testing, HIV/STI prevention education, STI syndromic management, ART and PrEP adherence counseling, care and psycho-social support group services and HIV related health care services at the community and facility level.
    • Support the integration of HIV/AIDS prevention and treatment services (Pre & Post exposure prophylaxis services, ART, Sexually Transmitted Infections, TB screening) to pregnant and breast-feeding mothers, and high-risk populations in line with current World Health Organization (WHO) recommendations and national guidelines both at the CSCC and through community ART teams and report performance indicators as appropriate.
    • Elicit for sexual partners from all clients newly enrolled and currently on treatment.
    • Contribute to the development and documentation of best practices, lessons learned and results and highlight the success of community based comprehensive HIV prevention, treatment and care among high risk populations.
    • Provide psychosocial support and trauma informed care and support, treatment, and referral services to high risk population.
    • Provide STI syndomic management to PLHIV on the field and in the CSCC.
    • Provide comprehensive triage (in-clinic and/or mobile) and clinical assessment service to clients resulting in safe and appropriate clinical decisions that reflect their healthcare needs.
    • Initiate appropriate nursing interventions (e.g. first aid and emergency interventions) to improve client outcomes and secure the safety of clients and clinic staff
    • Make recommendations for care utilizing approved SOPs and protocols
    • Where referrals for other healthcare services are necessary, support arrangements for the appropriate service via the appropriate referral system.
    • Deal professionally and sensitively with highly personal, delicate, confidential and sometimes complex issues
    • Create new medical records including clients’ treatment information and other relevant parameters
    • Document accurate and concise clinical summary that outlines reasons for visit, decisions reached, relevant client history and follow up appointments with timescale given to client.
    • Part of the retention and client management team (CMT) in monitoring adherence; working with clients and other health service providers to address difficulties
    • Carry out any assignment as may be decided by the clinician and/or CSCC lead.

    Qualifications and Experience

    • Must be a certified Nurse
    • A minimum of 2 years experience in HIV Programs.
    • Good communication skills (Oral & Written).
