    Medical Records Officer

    Job Summary

    • The Medical Records Officer is responsible for the accurate documentation, organization, and management of patient and health service records within the organization’s healthcare programs or facilities. The officer ensures that patient data is properly captured, securely stored, and readily accessible to authorized personnel for effective clinical care, monitoring, evaluation, and reporting.

    Key Responsibilities

    • Maintain accurate and up-to-date patient medical records in both paper and electronic systems.
    • Register new patients, update demographic information, and file medical documentation appropriately.
    • Ensure proper filing, retrieval, and archiving of health records for easy access and confidentiality.
    • Track patient visits, referrals, and follow-ups as required by program protocols.
    • Enter and update patient and service data into electronic health information systems (e.g., DHIS2, EMR, or other program-specific databases).
    • Verify completeness and accuracy of data before entry and submission.
    • Generate and share regular reports to support clinical and M&E needs.
    • Support the harmonization of patient-level data with program monitoring systems.
    • Work closely with clinicians, M&E staff, and program officers to ensure integrated data management.

    Requirements

    • Diploma or Bachelor’s degree in Health Information Management, or Medical Records
    • Valid Medical Records Practice Liscense
    • Minimum of 2–4 years’ experience in medical records management, preferably in an NGO, hospital, or donor-funded project.
    • Knowledge of health data systems such as DHIS2 and  EMR is an advantage.
    • Strong understanding of patient confidentiality and data protection standards.
    • Excellent organizational and documentation skills.
    • Proficiency in MS Office (Word, Excel) and basic database tools.

    Community Health Practitioner

    Job Summary

    • The Community Health Practitioner (CHP) plays a vital role in promoting, protecting, and improving the health of individuals and communities. The practitioner provides preventive, promotive, and basic curative services while educating community members on health practices that enhance well-being.
    • The CHP serves as a link between the health facility and the community, supporting public health initiatives, disease surveillance, and primary healthcare delivery.

    Job Details

    • Conduct HIV prevention, treatment, care, and support services, STI syndromic management, other related health services for vulnerable populations at designated communities
    • Conduct HIV testing services at outreach venues in line with the national guidelines, existing protocols, and standard operating procedures.
    • Counsel clients who tested positive to HIV and enroll them on treatment.
    • Provide adequate adherence counselling and ensure client are motivated to remain in treatment.
    • Ensure patient care provided at the facility and outreach is in accordance with National guidelines for clients.
    • To support health promotion and community mobilization to improve community knowledge on health issues and eventual increase in demand for services.
    • Be available for monthly and quarterly supervisory visit during fieldwork to ensure the provision of quality home base services to patients on treatments.
    • Prepare and submit weekly and monthly reports.
    • Motivate clients to take responsibility of their health and overall well-being.
    • Work to reduce cultural and socio-economic barriers between clients and the CHRCs.
    • Must be present every week at the facility to update all client details into ART care card and EMR.
    •  Must always be available for work- and work-related activities.
    • Must be able to carry out Cervical cancer counselling and Screening.

    Requirements

    • Diploma or Degree in Community Health PracticePublic Health, or related field.
    • Valid practicing license from the relevant regulatory council or board.
    • Minimum of 2–5 years of experience providing community-based health services.
    • Strong knowledge of primary healthcare, health education, and disease prevention.
    • Ability to engage with diverse communities and communicate health messages effectively.
    • Good recordkeeping and data management skills.
    • Fluency in English and local languages is an advantage.
