    ESM Pharmacist (Afikpo LGA)

    Job Description

    • The main responsibility will be to provide pharmaceutical care services and support medication adherence counseling services within the communities.

    Area 1: Delivery of pharmaceutical & commodity supply chain services in support of quality Antiretroviral Therapy (ART):

    • Maintain adequate health commodity supply chain both within the ESM site and the supported facilities within the senatorial zone.
    • Ensure efficiency and optimization of all the DSD Models
    • Dispense Antiretroviral drugs (ARVs) and other medication in line with approved regimens and standards.
    • Dispense ARVs in appropriate packaging with clear instructions.
    • Provide medication-related counseling, including reinforcing adherence and advising on potential side effects and drug interactions.
    • Provide drug information to health service providers and clients on request.
    • Identify and support follow-up of clients who do not return refills of their ARVs.
    • Work with providers and clients to identify, manage, and report adverse drug reactions and drug interactions.
    • Contribute to decision-making on treatment and care for individual clients.
    • Ensure Good Pharmaceutical Practice (GPP) and standards.
    • Calculate drug storage needs for ART start-up and expansion.
    • Monitor ARV and other drug (STIs, OIs, etc.) stock levels communicate with the project team for re-stocking.
    • Contribute to forecasting /quantification of health commodities and monitoring of drug distributions.
    • Oversee drug logistics i.e., stock-taking, proper drug storage, drug stock out and monitor the effectiveness of the pull system.

    Area 2: Internal and External Quality Control Systems:

    • Assess the functionality of existing pharmaceutical management systems and develop implementation plans that prioritize and address gaps.
    • Oversee quality assurance systems at the facilities by close monitoring of prescription patterns, ensuring proper documentation, side effects monitoring (pharmacovigilance) and reducing dispensing errors.
    • Identify needs for information resources—for example, ART guidelines and drug information for medication counselling.

    Area 3: Monitoring and Evaluation:

    • Monitor and report medication errors, suspected adverse reactions to medicines, and other medication-related events.
    • Monitor ART prescriptions for appropriateness and any discrepancies.
    • Monitor adherence to ARVs; working with clients and other health service providers to address challenges.
    • Address emerging problems as client numbers increase.
    • Monitor and report weekly enrolment.
    • Provide support to the Strategic Information (SI) unit to conduct research (qualitative and quantitative) surveys on service delivery outcomes for HIV prevention, care, and treatment.
    • Ensure timely submission of quality reports on a weekly, monthly, bi-monthly, and quarterly basis or as required.
    • Maintain transaction records and inventory management tools as required to ensure transparency in commodity management records.
    • Ensure timely and accurate reporting of logistics reports to facilitate stock replenishment and accountability.

    Area 4: Other assignments:

    • Participate in cluster coordinating meetings and represent the project at relevant fora.
    • Carry out any assignment as may be decided and delegated by the Supervisor.

    Minimum Recruitment Standard

    • A Pharmacist with at least 1 - 2 years’ experience in TBHIV management.
    • Must have a current practising license.Good Knowledge of computers with the ability in Microsoft Excel for simple data analysis.

    Integrated Service Nurse

    Job Description

    • The Integrated Service Nurse will facilitate the integration of comprehensive HIV/TB and other health services into existing outpatient, inpatient, and specialized care units, ensuring holistic and client-centered care delivery across supported facilities.
    • They will coordinate the activities of other ancillary staff for HIV service delivery, under the guidance of the TA clinical services and site staff at the health facility.

    Responsibilities

    • Support site level activities to embed HIV services into all relevant clinical units (outpatient, inpatient, antenatal, etc.).
    • Ensure seamless coordination between HIV/AIDS services and general health services.
    • Support the delivery of HIV testing services, adherence counselling, and ART initiation at facility and community service delivery points by connecting them to the relevant service providers within the facility or via facilitated referrals, to sustain treatment.
    • Facilitate diagnosis and treatment of co-infections and comorbidities.
    • Support PMTCT activities, including antenatal, intrapartum, and postnatal care.
    • Coordinate the delivery of health education and treatment literacy to PLHIV on relevant topics including ART, treatment adherence, U=U, and positive living.
    • Ensure service alignment and timely uptake of relevant services (e.g. TPT, MMD, DSD, Viral Load) to maximize client encounters, reduce service fragmentation and improve treatment outcomes.
    • Oversee pre-appointment reminders and tracking of missed appointments within 24 hours via phone, SMS, or home visits to ensure treatment continuity.
    • Support in the delivery of enhanced adherence counselling for clients with unsuppressed viral load.
    • Facilitate the uptake of health insurance enrollment for PLHIV in assigned health facilities
    • Perform additional duties as assigned.

    Minimum Recruitment Standard

    • Bachelor’s Degree in Nursing, or equivalent.
    • Current practicing license
    • Strong knowledge of HIV care, treatment, and prevention services.
    • Coordination and supervisory skills.
    • Effective communication skills in English and the local dialect.
    • Basic computer skills, including proficiency in Microsoft Word, PowerPoint and Excel.
    • Ability to work in multidisciplinary teams and manage competing priorities effectively.

    Health Informatics Officer

    Job Description

    • The incumbent will be engaged to ensure the optimization and full functionality of all supported databases (LAMIS inclusive) across all Lagos State ACE-supported facilities while also coordinating all Health Informatics-related activities in the State.
    • S/He will provide technical assistance for the design and content development for electronic information systems (LAMIS, DHIS, GIS, and any other package) used for M&E operations.

    Senior Finance Associate

    Technical Officer - Adult ART & TB / HIV

    Job Description

    • Will provide technical support to implement high quality care and support activities with primary focus on tuberculosis control and treatment as well as all issues surrounding TBHIV co-infection.
    • S/He will also provide support related to TB/HIV collaboration strategies and approaches, related to implementation of collaborative activities at the State Level, coordinate the design and implementation of components related to tuberculosis programs and TB/HIV collaborative activities in all implementing states, provide technical assistance in tuberculosis and TBHIV management capacity building to field programs and work in close collaboration with the Care and Treatment Unit in the implementation of integrated HIV, TB and other medical services at the facility level.

    • Possession of an MPH or post graduate degree in a related field is required.
