    Medical Doctor

    Main Duties and Responsibilities

    • Apply medical knowledge and skills to diagnosis and prevention. Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting MSF protocols.
    • Keep the patient and/or his/her family informed about the illness and provide appropriate explanations about the treatment to follow, checking they have understood.
    • Follow up on the evolution of the hospitalized/IDP patients, through daily visits, consultations and examinations, prescribing the necessary treatment following MSF protocols, deciding whether they can be discharged or transferred to other departments – in collaboration with other doctors- and informing their family about the patient’s evolution.
    • Check and control the rational distribution of medicines and equipment under his/her responsibility and take care of the quality, disinfection, and sterilization of the medical material. Assure general compliance with standing hygiène standards.
    • Participate in the collection and analysis of epidemiological data, checking its validity and informing the line manager or project coordinator about any problem or complication of the patient’s illness, or medical error and monitors the proper functioning of the department, equipment or material.
    • Ensure bedside training of the paramedical team in order to optimize the quality of care.
    • To work in collaboration with the nurses to ensure medical care of quality
    • To work as a team member, taking into consideration the non-medical problems the patients have to face (food and care problems etc.)
    • To respect medical ethics and confidentiality

    MSF Section / Context Specific Accountabilities
    On admission: ER/ICU:

    • To perform well the admission criteria triage criteria
    • To examine all new cases admitted in the ITFCs.
    • To carry out basic medical examinations.
    • To recognize, call and/or immediately inform the MAM/ Pediatrician and MD Senior about the children in a serious condition requiring emergency interventions.
    • To refer to MOH/Partners non malnourished children with complicated cases requiring further management.
    • To check the patient’s hydration status and to prescribe rehydration treatment if necessary.
    • To prescribe routine and basic treatment according to MSF protocols used in respecting the drug dosage based on the weight of the patients.
    • To Complete and properly fill the individual card with the medical data.
    • To give clear instructions to the nurses on the medical care and specific monitoring of the patient.
    • To do a quick handover round to be sure that the patients are ok for the next shift, the directives are done.
    • To attend medical and mortality review meetings

    Monitoring of the patients:
    In phase 1:

    • Daily follow up of patients in Phase 1 To give the medical treatment according to the protocols.
    • To estimate the medical evolution (improvement, deterioration etc.) and daily follow up of the patients to detect complications and refer either in ICU/ ICU Stepdown or Externally. To report to MAM/Paediatrician/ MD Senior the patients who are not gaining weight or who have complicated medical problem.
    • To manage properly the patient folder
    • To decide, in collaboration with the nurse, on the transfer of the patient(s) to phase 2.
    • To do a quick handover round to be sure that the patients are OK for the next shift, the directives are done.
    • To monitor feeding of patients in collaboration with other team members.
    • To prescribe basic medical treatment.

    In phase 2:

    • To decide, in collaboration with the nurses, on the discharge of the patients.
    • To ensure all patients above 6 months are vaccinated upon discharge.

    Minimum Required Skills and Qualifications

    • Education: Bachelor of Medicine and Surgery (MBBS) essential. Required license to practice
    • Experience: Minimum 2 years experience as a Medical Doctor or in clinical.
    • Previous experience in Nutrition/HIV/TB program, General Pediatric ward would be an asset.
    • Languages: English essential. Local language (Hausa, Fulani) is desirable.
    • Knowledge: Essential computer literacy (word/excel)

    Nurse

    Main Duties and Responsibilities

    • Knowing, promoting, implementing and following up the universal hygiene standards/ precautions, bio-hazard prevention and infection control, security rules and other protocols and procedures in the medical premises and ensuring high standards of hygiene of his/her working environment.
    • Respect medical secrets and confidentiality at all times.
    • Organizing and carrying out care and treatments according to medical prescriptions, and assisting during consultations, daily rounds and other medical procedures.
    • Participating in health education of the patient (and family) when necessary.
    • Identifying Sexually Gender Based Violence victims and referring them to the medical team so they can receive the necessary treatment. Knowing and is aware of the importance and appropriate use of the Post Exposure Prophylaxis (PEP) Kit
    • Ensure patients are received, installed and that those with lack of autonomy are assisted, especially regarding their feeding, personal hygiene, movements and comfort.
    • For ER and OPD nurse, ensuring triage of patients in waiting areas, wards or during emergencies, detecting the priority acute/emergency cases, carrying out first aid care and referring them to the doctor when necessary
    • Sending material to be sterilized and recuperates it from sterilization
    • Carrying out admission, surveillance and follow-up of patients (i.e. assessment of the evolution of their state of health and the identification of any emergency situation or deterioration
    • Carry and supervise administrative procedures and documents (fill in patient’s files, forms, consumptions, statistics, registers, health files, etc.), ensuring an appropriate written/oral handover, and reporting any problematic situations and cases that may arise. Participating in data collection and keeping doctors/supervisors informed.
    • Participating in the department-related pharmacy and medical equipment control and maintenance (quality and its functioning, storage conditions, follow-up of expired drugs inventories, stock takes of medicines and material at every team changeover ensuring no material is taken out of nursing area/wards without prior authorization.
    • Perform cleaning and minor maintenance for biomedical equipment used. Following the user manual and protocols and alert supervisor in case of malfunctioning.
    • Supervising and training nurse-aids, nutrition assistants, cleaners and other related staff under his/her supervision, ensuring teamwork and adherence to all appropriate protocols and procedures

    MSF Section/Context Specific Accountabilities
    Administration:

    • Ensure all the new admissions in the waiting area are seen and sort them according to our triage system to rapidly identify life threatening conditions in patients that must be treated immediately (priority1), those with serious conditions that should be treated rapidly (priority2) and those who should be treated in turn (priority 3).
    • Regular monitoring of general status of patients, giving priority for consultation to those that present with signs of severity such as; lethargy-unconsciousness, convulsions, fever, dehydration, respiratory distress and malnutrition.
    • Provide routine nursing care to ATFC and ITFC patients as per Dr’s prescription and NTS recommendation, or specific cares driven by finding from routine patients monitoring activities.
    • When called upon, together with the ER team, be able to assist in resuscitation of critical patients.
    • Lead patients to their respective department decided by the MD and give an in-depth handover to the nurses there.
    • Support in data collection by proper filling of registers, tally sheet, etc. as assigned by the supervisors.
    • Maintain adequate IPC measures during all cares and ensure correct waste segregation and disposal according to MSF guidelines- respect the bucket system in place.

    Patient orientation:

    • Participate in health education of the patient (and family) when necessary while checking the admission checklist.

    Teamwork and cooperation:

    • Participate in meetings organized by the department or supervisor.
    • Participate/go to mandatory trainings requested by the supervisor.

    • Experience: Minimum of 2 years’ experience in Nursing practice with MSF, other INGO/NGO or Private medical facility.

    • Knowledge: Average knowledge of malnutrition case management protocols.

    Nurse
