    Medical Doctor

    NIGERIA – Medical Doctor(3)MISSION AND MAIN ACTIVITIES

     Serve as a dedicated medical doctor specializing in the admission, assessment, and ongoing care of children presenting with Severe Acute Malnutrition (SAM) and pediatric patients aged 0 to 59 months, which encompasses the crucial developmental stage up to 5 years of age. In this role, you will play a pivotal role in evaluating and managing the health and well-being of these young patients, providing them with comprehensive medical attention and personalized treatment plans tailored to their specific needs. Your expertise will contribute significantly to the early identification, intervention, and recovery of children facing nutritional challenges and other pediatric health concerns, ensuring that they receive the highest standard of medical care during this critical phase of their growth and development.

    Main Responsibilities:

    • Work as a medical doctor for admission and review of SAM and pediatric patients (0-59 months) or below 5 years
    • Apply specialized medical knowledge and skills to diagnose and prevent illnesses in pediatric patients under the age of 5. Conduct both outpatient and inpatient consultations, adhering to ALIMA protocols at the designated duty station.
    • Communicate clearly with young patients and their families about their health conditions, providing age-appropriate explanations of the recommended treatments. Ensure that parents or guardians have a comprehensive understanding of the available treatment options and potential outcomes
    • Monitor the progress of hospitalized pediatric patients through daily visits, consultations, and examinations. Prescribe treatments in line with national guidelines and ALIMA protocols, make decisions regarding discharge or transfer to other pediatric departments while collaborating with fellow pediatricians, and keep families informed about the child's progress.
    • Maintain accurate and comprehensive patient records, documenting the final diagnosis at the time of discharge, which will be recorded in the praxis system.
    • Participate in continuous training sessions with the multidisciplinary medical team to enhance the quality of care provided to pediatric patients.
    • Adhere to and implement all ALIMA medical protocols, ensuring strict compliance with universal precautions at all times. Mitigate bio-hazard risks and enhance infection control practices. Safeguard the confidentiality of patients’ medical information.
    • Assist or perform emergency pediatric procedures, such as minor surgical interventions, under the supervision of senior pediatricians, while adhering to ALIMA’s protocols. Accurately document operative notes following each procedure.

    EXPERIENCE AND REQUIREMENTS

    • MBBS with full registration with the Medical and Dental Council of Nigeria.
    • Experience with medical international NGO, an asset
    • Experience with pediatric patients below 5 years is an asset
    • Team management and supervision skills
    • Perfect knowledge of MS Office packages, especially Excel & Word
    • Oral and written English mandatory, fluency in Hausa essential

    MHPSS Psychologists

    NIGERIA – MHPSS Psychologists (3)

    MISSION AND MAIN ACTIVITIES

     Provide specialized services and individual psychological support for people(caregivers) with moderate to severe mental and/or emotional disorders in the different areas of intervention. Provide technical support to Health staff to implement sound psychosocial support (PSS) activities for children and adults. The Psychologist is responsible for providing psychotherapy services to beneficiaries in need of MHPSS services to ensure quality of mental health and psycho-social services (MHPSS) in the fields

    Main responsibilities: 

    • Assist cases with identified mental or emotional disorders referred internally or externally
    • Conduct case-by-case needs assessments for the diagnosis of psychological disorders and identification of treatment and therapy
    • Carry out evaluation of mental health needs of patients/beneficiaries and establishing a diagnosis and a plan of treatment following WHO MHGAP approach and ALIMA case management protocols. Referring cases to other health professionals whenever necessary, in order to provide the most appropriate treatment and improve the patient's condition
    • Providing psychological support and psychotherapy sessions (individual, family, group) to beneficiaries in need according to ALIMA, MHGAP and other standards protocols in order to improve their psycho-social and mental health conditions
    • Document progress on cases, maintaining strict protocols for confidentiality and consent, and ensure that data protection protocols are respected
    • Assisting and training staff members (Nutrition and health) and resourceful people whenever necessary in order to enhance the scope of MHPSS activities. Facilitate trainings for internal staffs and communities on psychosocial first aid (PFA)
    • Strengthen community based psychosocial support and peer support group activities standard protocols and procedures set in order to provide the most appropriate management.
    • Provide coaching and support to the field staff in the implementation of structured MHPSS activities
    • Carry out all tasks respecting key protection principles, including but not limited to: do no harm, non-discrimination, confidentiality, accountability and need-to-know principles
    • Establish and maintain collaborative relations with relevant partners and other project stakeholders working in the project areas
    • Proactively participate in relevant coordination meetings, as requested by the coordination

    EXPERIENCE AND SKILLS

    • Bachelor’s degree or equivalent in social work, mental health, psychology or related fields
    • Minimum of 2-4years of specialized experience in community based psychosocial support interventions particularly as it relates to social norms change.
    • Experience with medical international NGO, an asset
    • Experience in a similar position for at least 2 years.
    • Demonstrated experience on counseling skills and clinical supervision of lay counsellors
    • Track record of developing technical curriculum and materials for delivery of MHPSS programming/activities in low resource setting

    Mental Health and Psychosocial Support Manager(MHPSS)

    MISSION AND MAIN ACTIVITIES

    Under the daily guidance of the Medical Referent and technical support of the Roving MHPSS Manager for Nigeria, the MHPSS Manager will provide overall technical and programmatic oversight of ALIMA’s MHPSS interventions in Katsina State. The MHPSS Manager will lead the process in development, implementation, and coordination of all MHPSS activities and provide coaching and mentoring to MHPSS Psychologists

    Main responsibilities: 

    • Coordinate and ensure that mental health activities with patients are performed with regular follow-ups, and ensure that the mental health team remains calm neutral
    • Identify the mental health support needs in the target population and participate in defining, planning, organising and updating the appropriate ALIMA mental health activities
    • Map and update the details of existing mental health services, with other actors.
    • Ensure, collaboration with the entire medical departments for identification of MHPSS clients.
    • Ensure the implementation of all ALIMA standard mental health protocols and procedures
    • Manage, train and supervise the mental health team through activities such as defining job descriptions, capacity-building,
    • Collect key client data and prepare reports on progress made, together with observations, challenges faced, and lessons learned.
    • Identify knowledge gap and give update for training accordingly.
    • Effective supervision and management of MHPSS team
    • Participates in external meetings and reports relevant information
    • Ensuring an appropriate MHPSS approach in the program aligned with the IASC MHPSS guidelines for emergencies and humanitarian settings.
    • Leading development process of referral pathways
    • Comply with all policies and protocols of the ALIMA agency
    • Other duties as assigned.

    EXPERIENCE AND SKILLS

    • Master’s degree or equivalent in social work, mental health, psychology or related fields or a combination of bachelor’s degree with significant work experience (3years+) in the mentioned fields
    • Minimum of 3-5years of specialized experience in community based psychosocial support interventions particularly as it relates to social norms change.
    • Experience with medical international NGO, an asset
    • Experience in a similar position for at least 2 years.
    • Demonstrated knowledge of IASC guidelines on MHPSS in emergency settings

    Nurses

    NIGERIA – NURSE (4)

    MISSION AND MAIN ACTIVITIES

     Oversee the overall well-being of the child and promptly address various medical issues. Detect and respond swiftly to critical medical concerns in children, promptly notifying the physician of serious cases. These include conditions like hypo- or hyperthermia, tachycardia, arrhythmia, respiratory distress syndrome, dehydration, diarrhea, vomiting, refusal to eat, severe anemia, edema, wounds, and pain. Regularly assess vital signs according to the child's condition and record all findings, including temperature, heart rate, respiratory rate, saturation, diarrhea, vomiting, edema, etc., on the monitoring sheet. Continuously monitor and revise the rehydration protocol for dehydrated children. Administer necessary treatments as required.

    Main Responsibilities:

    • Medication Preparation and Administration:
    • Prepare prescribed medications, adhering to specified dosages.
    • Prior to administering medications, verify the card number and the child's bracelet for accuracy.
    • Employ aseptic nursing techniques to ensure the child's safety and well-being.
    • Create a conducive environment by calling the child and ensuring comfort for both the mother and child during treatment.
    • Administer prepared medications by crushing tablets and diluting them with a small quantity of water.
    • Ensure the child consumes the complete prescribed dose. If vomiting occurs within 10 to 15 minutes post-administration, re-administer the same dose.

    Injection Procedure:

    • When administering injections, adhere to precise injection site guidelines and employ rigorous aseptic techniques.
    • Maintain accurate record-keeping by documenting the injection procedure on the processing sheet.

    ORS Administration:

    • Administer and regularly update the appropriate quantity of Résomal (or ORS for infants and moderately malnourished children) in accordance with the established protocol.

    Fluid Infusions and Transfusions:

    • Regularly assess and adjust flow rates for infusions and transfusions every 15 minutes as needed.
    • Adhere to established protocols for monitoring blood transfusions and infusions, promptly reporting any irregularities.
    • Document fluid administration on the child's monitoring sheet.

    Dressing and External Care:

    • Perform dressing changes following nursing protocols.
    • Ensure proper placement and care of nasogastric tubes (SNGs), with daily attention and replacement every 3 days.
    • Maintain strict hygiene and universal precautions during care.
    • Consult children in different phases, including intensive care, and follow consultation schedules.

    Clinical Examination and Care Decision:

    • Conduct thorough head-to-toe clinical examinations, considering medical history, treatment, and weight trends.
    • Make informed decisions based on observed signs and symptoms, respecting national protocols.
    • Determine necessary laboratory tests, monitoring frequency, treatment adjustments, and potential consultations with the doctor.
    • Evaluate the need for dietary changes following national nutritional guidelines.

    Nutritional Monitoring and Management:

    • Monitor children's nutritional status and understand admission, referral, transfer, and exit criteria as per national protocols.
    • Calculate target weight using Z scores and validate weight curve accuracy.
    • Inform mothers about their child's progress and provide explanations about the care process.
    • Oversee morning weighing sessions and guide diet changes while adhering to nutritional protocols.
    • Supervise nutritional assistants and ensure adherence to meal and hygiene guidelines.

    Hygiene and Equipment Oversight:

    • Maintain hygiene standards in different care areas, including ICU and Pediatric Ward.
    • Verify drug availability and equipment integrity.
    • Ensure proper equipment storage and maintenance, reporting damages as needed.
    • Conduct daily checks of resuscitation equipment in the emergency room and update missing items.

    Patient Education and Comfort:

    • Educate mothers about children's care procedures and provide reassurance during treatments.
    • Participate in health education for companions, emphasizing proper food and hygiene practices.
    • Assist mothers with care when necessary to support their understanding.

    Team Communication and Handover:

    • Facilitate smooth transitions between teams through effective oral and written handovers.
    • Provide comprehensive oral handovers during team changes, addressing specific child concerns with the relief nurse.
    • Document all significant events during the shift in written handover reports.

    Facility Maintenance:

    • Maintain cleanliness and order in the care environment, ensuring proper equipment storage and sanitation at the end of each service.

    Team Leadership and Care Oversight:

    • Lead and motivate nurse aids and hygienists on a daily basis, ensuring optimal performance and adherence to care standards.
    • Provide formal and informal staff training, in coordination with hierarchical referents, to enhance team competence and skills.
    • Maintain oversight of care quality to ensure consistently high standards.

    Monitoring, Analysis, and Reporting:

    • Actively engage in monitoring and reporting medical activities, including accurate completion of patient records, forms, statistics, and reports.
    • Offer insightful analysis of gathered statistics to contribute to informed decision-making.
    • Facilitate effective communication by ensuring correct information transfer to the next service team.

    Timely Problem Reporting:

    • Swiftly communicate any service-related issues, including medical facility loss, theft, or damage, to the technical referent for immediate resolution.

    Adaptability and Assigned Tasks:

    • Undertake additional tasks as directed by the direct supervisor, aligning with the position's objectives and contributing to overall effectiveness.

    EXPERIENCE AND REQUIREMENTS

    • Essential: recognize Nursing Diploma/Degree
    • Essential: Registration with the Nursing and Midwifery Council of Nigeria
    • Essential: 2 years experience in a similar position in a Medical organization preferably INGO
    • Previous experience as Nurse with ALIMA or another INGO is desirable
    • Fluency in Hausa essential. Oral and written English language mandatory.
