    Facility Case Manager (Embedded in the Treatment Facilities)

    Responsibilities
    Facility Case Managers are responsible for:

    • Facility Saturation: FacilityCase Managers are to work with facility focal persons at specific service delivery points, namely ANC (PMTCT), Immunization clinic, ART Clinic, Social Welfare, TB clinic, Pediatric Ward and Nutrition clinic to facilitate the identification of HIV positives children (CALHIV) for immediate enrollment into the ICHSSA-4 project.
    • Index case elicitation (ICE): Facility Case Managers are to support index case elicitation, especially on ART clinic days, to ensure children of HIV positive adults (PLHIV) are identified and linked to HIV testing services. Newly identified positive cases are to be enrolled unto the project as children living with HIV (CALHIV), while children testing negative are also to be enrolled unto the project as children of persons living with HIV.
    • Client care audit (CCA): Facility Case Managers are to track gaps in service provision i.e., all treatment services that are yet to be received by the client, including uptake of
      • Tuberculosis screening and tuberculosis prophylaxis (IPT)
      • GBV screening and referral to GBV Unit
      • Pregnancy screening and referral to ANC
      • Same day initiation on anti-retroviral medication
      • Correct drug Regimentation by age
      • Placement on multi-month drug refill (MMD3 or MMD6)
      • Viral load sample collection and referral for enhanced adherence counselling.
      • Facilitate all Facility- level program activities
    • Provide documentation support along the 95-95-95 cascade: Facility Case Managers are to work closely with case managers to populate the ICHSSA-4 95 cascade templates correctly and accurately, which serves to capture HIV indicators.
    • Tracking back of LTFU: Facility Case managers are to support client tracking for defaulters and lost to follow-up clients through the generation of lost to follow up list for commencement of tracking back to care. Tracking is to be graduated with CALHIV as priority, then female aged 25-49, then others.
    • Facilitate all Facility- level program activities: These included OTZ meetings, support group meeting as avenues to elicit and enroll CALHIV aged 0-17years, Quarterly facility referral coordination meetings for data reviews.
    • Provide documentation support along the 95-95-95 cascade: Facility Case Monitors are to work closely with case managers to populate the ICHSSA-4 95 cascade templates correctly and accurately, which serves to capture HIV indicators.

    HIV Integrated Care and Support Officer

    Job Summary / Responsibilities

    • S/he is to provide technical guidance to PHI/ICHSSA 4 partner CSOs in the implementation of technical program activities for community-based HIV Care and Support in the ICHSSA 4 Program.
    • Under the direction of theproject Care and Support Advisor, s/he will collaborate closely with the ICHSSA team to roll out and implement an integrated and holistic community-based response to HIV/AIDs towards scaling up access to HIV prevention and care services for vulnerable children and their caregivers
    • Provide technical input for the development of strategies, design, and implementation of HIV Prevention/ Care & Support activities by ICHSSA 4 project; ensure activities improve access to HIV prevention and care services amongst project beneficiaries and their caregivers.
    • Direct CSOs towards overall technical quality - including adherence to technical standards, best practices and donor guidelines for all HIV Prevention Care & Support activities of the project.
    • Support strategies towards ensuring that project activities are built on strong behavior change principles, and build the capacity of CSOs and relevant GON staff in effective implementation and monitoring.
    • Work closely with the SMOH, SMWASD, SACA and HIV TWGs in Zamfara state in identifying and capitalizing on leverage points for improved linkages and referral coordination at the community level and for integration of HIV Prevention Care & Support elements into other OVC areas project activities and vice versa

    Qualifications and Skills

    • MBBS, Master of Science and/or MPH with a minimum of 3years relevant experience in implementing HIV Prevention, Care and Support programs or BS/MA in a relevant health degree or Social Sciences, plus a minimum of 3 years technical experience with community-level HIV prevention, Care & Support programs
    • Demonstrated ability to work effectively with government and civil society partners and communities as well as transfer knowledge to diverse audiences through workshops, training and meetings using participatory methods and adult learning principles. ·
    • Experience in supporting HIV Testing and Counselling programs in health facilities, mobile units and community outreaches
    • Good knowledge of Government of Nigeria HIV Care and Treatment guidelines (Pediatric & Adult) and experience working with Ministry of Health and other relevant entities
    • Familiarity with PEPFAR-OVCprogramming and principles
    • Excellent English oral and written communication skills required.
    • Excellent analytic, organizational and computer skills; skilled in MS Office including Word, Excel, Outlook, and PowerPoint.

    Case Monitor

    Job Description
    Case Monitors (Responsible for the Facility-Community Bi-directional Linkages and Directly Supervised by the ICHSSA 4 Staff)

    Case Monitors are solely responsible for:

    • Technical and programmatic leadership in the implementation of OVC interventions across the facility – community spectrum.
    • Supervise and give technical oversight to all cross-cutting linkage interventions by facility case managers and community case workers.
    • S/he manages and coordinates case conference among case workers to address concerns and issues arising from service provisions
    • Conduct initial comprehensive assessment of clients’ needs and support systems using the National OVC service standards and assessment tools
    • Supports the training of peer navigators, case workers on the care process for project beneficiaries
    • The CM is expected to strengthen community and health facility linkages and bidirectional referrals to optimize care, treatment and support services to vulnerable children infected or affected by HIV.
    • The CM will support training of case workers on gender norms, adolescent, nutrition, child protection programming.
    • The CM will interface on all spectrum of care for project beneficiaries and provide community level support to them including early childhood and parenting services.
    • Collaborate with Facility ART team to ensure CALHIV/ALHIV and PLHIV (Caregiver) is on optimized regiment and retained on treatment.
    • The CCM will provide support in the identification, linkages for Adolescent girls, women for support in SVAC and GBV response systems in the state.
    • CCM will participate in monthly case conferencing with PHI and conduct same with case workers at the CSO level.
    • CCM will collate, coordinate monthly reports of activities across the different thematic areas support, provide a summary of areas of challenges and analysis aimed at providing next line of interventions.

    Primary Responsibilities

    • Provide assisted referral services and linkage to care and treatment services for test indicated children.
    • Supports adolescent OTZ clubs, adolescent programs, and community-based programming platforms such as Kids Clubs, Caregivers’ Forum that are usually facilitated by Case Workers.
    • Collaborate with facility case finding team to ensure newly identified HIV positive clients are properly linked to treatment.
    • Support research efforts including assessments using instruments developed for information gathering on the ICHSSA 4 project.
    • Collaborate with Facility ART team to ensure CALHIV/ALHIV and CA/PLHIV (Caregiver) are on optimized regimen and retained on treatment.
    • Develop and roll out mentoring, coaching and supportive supervision program for community case workers on service delivery to key population
    • Support the updating of viral load result through collaboration with health facilities staff
    • Monitor the implementation of care plans for each household in their community to ascertain the progress being made and the level of support being provided by the Community Case Workers.
    • Strengthen pediatric ART Saturation Support (PASS) at households and community levels in collaboration with the Peer Navigators.
    • Perform any other functions that might be assigned from time to time depending on emerging needs and priorities of the project.

    Qualifications

    • Candidates should possess an HND / B.Sc Degree with 1 - 3 years relevant work experience.
