    Community Health Nurse - Abuja

    Mission Objective

    • The REACH program is dedicated to closing healthcare gaps by bringing quality and affordable services directly to communities. 
    • The community health nurses deliver superior community and home-based healthcare services, prioritizing prevention, promotion, and basic curative care within designated clusters.     Duties And Responsibilities

    PURPOSE

    • Perform patient assessment using appropriate, problem-focused, and age-specific assessment techniques. Analyze the assessment data, derive nursing interventions, set priorities according to patient needs, formulate a plan of care, implement and evaluate the outcome.
    • Provide emotional support and measures to alleviate fear and anxiety. Assess patient and family readiness and identify learning needs. Participate in voluntary community health activities to promote, maintain and restore health and prevent diseases.
    • Provide basic life support when needed. Document all relevant data in the electronic medical record according Administer medications, monitor desirable and undesirable effects, and intervene appropriately.

    DELIVERY

    Targeted Specialized Knowledge 

    • A minimum of Nursing certificate
    • Active membership of Community Based Organizations (CBO)  and/or work within the private health sector is highly desirable. 
    • Must come from the assigned community and must be able to communicate proficiently in appropriate language of the assigned cluster.
    • Hold and maintain a current nursing license to practice in Nigeria.

    Facility Manager

    Mission Objective

    Pharmacist - Lagos

    MISSION OBJECTIVE

    DUTIES AND RESPONSIBILITIES

    BASIC TRANSACTIONS PROCESS 

     PURPOSE

    • Provide effective medication use and become the people’s trusted ally against health issues that way, you can deliver maximum value to both the pharmacy and society. Review and develop the clinic's pharmacy policies and regulations.
    • Participate in voluntary community health activities to promote, maintain and restore health and prevent diseases. Manage clinical risk.

    BASIC INFORMATION ORGANIZATION

    DELIVERY

    • Prepare and dispense medications to patients on physician’s order. Organize the pharmacy in an efficient manner to make the identification of products easier and faster.
    • Maintain an up-to-date inventory of the clinic drug formulary. Records of all drugs, doses, and prices on the automated inventory of the clinic. Review, analyze and respond to different situations encountered daily.
    • Review and execute physician’s prescriptions checking their appropriateness and legality. Maintain full control over delivering, stocking, and labeling medicine and other products. Develop effective medication plans to minimize the risk of adverse side effects.
    • Maintain pharmacological knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; and participating in professional societies.

    TARGETED SPECIALIZED KNOWLEDGE

    • Minimum qualification of Bachelor's degree in Pharmacy/M.Pharm/PharmD Minimum of two (2) years post NYSC progressive experience in Pharmacy within the private and public sector. Experience working with public health and health systems is an advantage.
    • Candidates must be a registered member of Pharmacists Council of Nigeria (PCN) with an up-to-date practicing license

    PURPOSE

    • Develop processes, subject-matter guides, playbooks, SOPs and workflow documents as required.
    • Develop learning materials and deliver in-house trainings within the role’s technical field of work.
    • Collect, analyze, and visualize project, business, and operational data as required specifically for targeted communication and reporting requirements.                        

    ENGAGEMENT

    • Work closely with program managers, project managers, and other project support functions to monitor, facilitate, and assure compliance with REACH project management method, requirements, and standards at all times during project delivery (active support and problem-solving function).
    • Collaborate with the communications team to align website content creation, blogs, newsletters, marketing materials / email, presentations and pitch decks.
    • Regularly prepare a variety of technical, management, activity and status reports for a number of internal and external stakeholders.
    • Recommend changes to processes to improve alignment of services with business/program objective.         

    DELIVERY

    • Timely production of contractual deliverables for clients (monitoring and control function).
