- Part II Fellowship Examination of West African College of Surgeons & Physician as well as National Postgraduate Medical College equivalents in the specified specialties;
- Current practicing license of the Medical & Dental Council of Nigeria; and
- National Youth Service Corp (NYSC) discharge certificate.
