We are seeking a compassionate and detail-oriented Care Coordinator to join our hospital team. The Care Coordinator will play a key role in ensuring seamless patient care by coordinating medical services, facilitating communication between healthcare providers, and assisting patients in navigating their treatment plans.
Key Responsibilities:
Act as a liaison between patients, doctors, nurses, and other healthcare professionals.
Develop and implement personalized care plans to ensure continuity of care.
Assist patients with scheduling appointments, referrals, and follow-up care.
Educate patients and families about treatment options, medications, and healthcare resources.
Monitor patient progress and collaborate with medical teams to adjust care plans as needed.
Ensure compliance with hospital policies and healthcare regulations.
Maintain accurate and up-to-date patient records and documentation.
Qualifications & Skills:
Bachelor's degree in Nursing, RN,RM
Previous experience in patient care coordination or healthcare administration is an advantage.
Strong communication, organizational, and problem-solving skills.
Knowledge of Nigerian healthcare systems and hospital procedures.
Proficiency in medical record management and patient care software.
