    Infection Prevention & Control Supervisor

    Background

    • We have been working in Nigeria since February 1996. A Memorandum of Understanding between Medecins Sans Frontieres and the Federal Government of Nigeria facilitates this.
    • We are at present co-operating with the Federal Ministry of Health, State and Local Government departments of health on various health projects.

    Main Objective and Responsibility of the Position

    • Implementing and supervising an infection control program in a project or health facility, according to MSF protocols, safety and IPC measures, in order to ensure quality of care and safety of patients, caretakers and staff.

    Accountabilities

    • In collaboration with each service activity manager, maintaining and supporting the day-to-day implementation of the standard precautions in health structures in all departments including sterilization, laundry and kitchen, and providing technical supervision, training and support to the staff on IPC related issues using existing tools such as audits, checklist, surveillance
    • Implementing and monitoring adherence to additional precautions for air, droplet and contact transmissible diseases when indicated.
    • Based on a given patient-diagnosis, evaluating the need and supporting the implementation, if needed, for additional precautions in collaboration with the medical team and act accordingly
    • Implementing as planned, the correct ‘clean and dirty flows’ inside the health facility. Ensuring that environmental standards in the facility are respected (bed spacing, number of handwashing stations etc.) and together with the Hospital Facilities Manager and WHS Supervisor, that the essential requirements are available in the health structures
    • Guaranteeing compliance to aseptic care techniques and to the correct use of antiseptics, disinfectants and detergents.
    • Checking the consumption and the quality of IPC materials and equipment, including cleaning materials and equipment, PPE, etc.
    • As member of the Infection Control Committee, participating in and supporting the development and following up the Infection Control Action Plan.
    • Supervising the team under his/her responsibility and participating in the associated HR processes (e.g. recruitment, training/induction, evaluation, potential detection, coaching, development and communication) in order to ensure both the sizing and the amount of knowledge required.
    • Reporting any irregularities to NAM or IPC managers or PMR and producing reports with information recommendations for the monthly medical reporting
    • Note: IPC standard precautions include Personal Protective Equipment (PPE), hand hygiene, prevention of accidental exposure to body fluids, safe and appropriate treatment of re-usable medical devices, waste management, cleaning and disinfection of surface and environment, respiratory hygiene and individual hygiene for staff, caretakers and patients.
    • IPC essential requirements include water supply, solid waste and waste water treatment/disposal, latrines and shower facilities, dead bodies management, vector control and medical waste management

    MSF Section / Context Specific Accountabilities:
    The IPC Supervisor in Noma Project is the main responsible for the implementation of all IPC-related activities, protocols and different recommendations to ensure the quality of care and patient safety in MSF Health Facilities including Noma Children Hospital Sokoto. The IPC Supervisor works closely with all multidisciplinary team members to raise awareness and ensure IPC issues are identified and addressed by all staff members.
    As a member of the Infection Control Committee, participating in and supporting the development and follow-up of the Infection Control Action Plan and other communication activities are key (may support MTL with minutes/agenda/action points, also may chair the meeting in absence of MTL)

    • Liaison with all Managers/Supervisors and Balanced Leadership and Communication Skills:
      • Liaises with Watsan Supervisor, to provide technical support to Hospital Cleaners in NHCS and ensure the cleanliness of wards (based on IPC requirements) and hospital yard. Provide support to the ITFC ward when needed.
      • Liaise with Watsan Supervisor to directly provide technical support to the laundry and ensure the quality of bed sheets and linens are in enough quantity to maintain adequate recycling.
      • In collaboration with WatSan team, ensure maintenance of a clean and hygienic environment.
      • In conjunction with Supervisors/Managers, carry out training of all staff (Medical, Lab, Hygiene Agents/cleaners, Watchmen etc.) on relevant IPC information - IPC Pillars and aseptic technique.
      • In conjunction with WatSan and Matron of the NCHS ensure IPC measures in the Kitchen are adhered to and maintained.
      • Liaise with the WatSan Supervisor to ensure vector control in the hospital wards and other departments (use of light insecticide etc) and dead body management in the ward while waiting for evacuation.
      • In Conjunction with WatSan Supervisor and Nursing Team Supervisor ensure waste segregation and collection in different wards and keep high hygiene standards.
      • Supervise Implementation of IPC measures in the base offices and support during emergency response.
    • Stock Management:
      • Under leadership of Nursing Activity Manager and Pharmacy Supervisor/technician ensure ordering, storage and management of IPC materials (hand sanitizers, surfanios and hexanios, chloramine solutions, face masks, disposable gowns etc) for daily usage.
      • Supervision & Policy /Protocol Implementation:
      • Policy management of IPC activities in all locations and work in conjunction with all heads of departments to ensure and supervise implementation of IPC policy and protocols.
      • Support the OT nurse and ensure surgical materials are safe for patients, sterilization procedures follow IPC protocols, and provide necessary training.
      • Ensure compliance of the 3 IPC pillatransmission-basedCleaning and Disinfection, transmission based precaution) in Noma Children Hospital sokoto and ITFC.
    • Reporting:
      • Be responsible for monitoring of hand hygiene compliance using the hand hygiene observation tool (HHOT) and monthly updating of results.
      • Carry out SIPCA assessment with support from line manager.
      • Report IPC activities in the MMR.
    • Other: Gap fill WatSan Supervisor /Manager in case of absence

    Requirements (Qualifications / Experience / Knowledge)
    Education:

    • Nurse or Midwife (or equivalent) Diploma.

    Experience:

    • At least two years’ experience working as a Nurse, midwife, or equivalent diploma.
    • Preferable: Previous working experience working with MSF or other NGOs
    • Experience in OT, Intensive Care Unit is an asset

    Languages:

    • English is essential, Hausa is desirable

    Knowledge:

    • Essential computer literacy (word, excel and internet).

    Project Coordinator Assistant

    Main Objective and Responsibility of the Position

    • Provide support to the Project Coordinator in the administrative part of coordination tasks, ensuring a smooth relationship with local and national authorities, contributing to the context analysis and follow-up, and providing translations and interpreting in order to ensure the smooth running of the project.

    Accountabilities

    • Monitor and report on key issues (security, risk, etc.) to the Project Coordinator and provide support in analysing contextual information, through a sound knowledge of counterparts, in different administrations in order to improve decision making processes.
    • File and update field contacts and correspondence (with other NGOs, UN agencies, local authorities, media, etc.) in order to ensure information sources accuracy and its availability on regular basis.
    • Assist the Project Coordinator in the drafting of correspondences with authorities and counterparts (government officials, UN agencies, NGOs, etc.) in order to ensure fluent and accurate communication flows.
    • Assist the Project Coordinator in Internal and External Communication sessions and events about MSF (for National staff, the Ministry of Health staff, as well as local authorities) in order to improve awareness and public opinion adhesion.
    • Ensures the continuity of relations with local actors and keeps updated the list of contacts.
    • Organize advocacy contacts and appointments for the Project Coordinator with local authorities and partners to ensure the continuity of relations with local actors and involved counterparts.
    • Execute clerk-related activities as well as prepare and conduct briefings on the project context to new employees in order to facilitate their integration onboard and within the project environment.
    • Translate documents and act as an interpreter when needed.

    MSF Section / Context Specific Accountabilities

    • The MSF project in Gummi is a new project. MSF, as a medical organization, will work on mainly primary health care (malnutrition and Integrated community care management). As the project is a new project in the area, the activities will evolve. The project coordinator will need to be flexible in terms of activities and in the geographical area of work.
    • The PC assistant develops and maintains the stakeholder’s network together with the Project coordinator in order for MSF to have the necessary contacts to insure the activities.
    • The PC assistant follow the news and the different context update and inform the PC.
    • He provides support analyzing contextual information, through a sound knowledge of counterparts, in different administrations in order to improve decision - making processes is provided
    • The PC assistant does oral and/ or written translation when needed and provide cultural / local information when needed.
    • Conduct cultural briefings on the project context to new employees.

    WatSan Supervisor

    Background

    Main Objective and Responsibility of the Position

    • Supporting the Water, Hygiene and Sanitation (Watsan) Manager in the implementation and supervision of Watsan activities, including tools and materials employed according to MSF standards and protocols, in order to improve health and living conditions of the target population.

    Accountabilities

    • Ensuring the day-to-day implementation and administration of assigned Watsan activities at project level, including but not limited to, water supply, excreta disposal, waste management, vector control, and dead bodies management
    • Ensuring that the MSF procedures and protocols are followed by the Watsan teams to guarantee the operational quality of the project;
    • Ensuring the maintenance and repair of technical equipment in the project to guarantee an adequate running of Watsan activities;
    • Ensuring the distribution of the materials and tools used in water treatment, hygiene and sanitation (e.g. physical organization and inventory of stocks, receiving and processing orders for water, sanitation and hygiene material, check that the amounts received are recorded, and check monthly consumptions, etc.)
    • Supporting the Watsan Manager in ensuring an appropriate emergency preparedness and response capacity (physical verification of stocks, contacts, transport means, staff training). Ensuring appropriate assistance to the project response team and if required, participating in emergency activities or exploratory visits.
    • Supporting the Watsan Manager in the team planning (e.g. staff rosters and admin, HR processes, etc.) and informing and involving the Watsan Manager in case of any major management or technical issue, and providing all required reporting
    • Planning and supervising the HR processes (recruitment, training, performance evaluation, tasks definition and internal and external communication) of the staff under his/her responsibility in order to ensure both the sizing and the amount of knowledge required for the activity
    • Participating in data collection and reporting as required.

    MSF Section/Context Specific Accountabilities:

    • Management of technical WatSan components in the hospital, guesthouse warehouse and support for ITFC; specifically with a focus on sanitation, water supply, hygiene and vector control. Participates in elaborating WatSan strategies and response plans including resources in collaboration with the medical and logistical teams.
    • Provides support to the logistical and medical teams in identification, implementation and monitoring of the Water, Hygiene and Sanitation activities in and outside health structures according to the protocols of MSF (e.g. essential Water and Sanitation requirements).
    • Collaborating with Ministry of Health to ensure the hospital facility and departments and compound are clean at all times using MSF protocols. This includes support for the ITFC.
    • Ordering and management of WatSan materials according to need, ensuring tracking of consumption and reporting on monthly basis.
    • Support the Health Promotion team in hygiene promotion activities in the health facility.
    • As member of the Infection Prevention Control Committee, participating in and supporting the development and following up the Infection Prevention Control Action Plan. A particular support might be required to assure that environmental control measures are defined to address infection control issues related to patients with highly infectious diseases.
    • Data Reporting: Ensure timely data report for Health Information System, LRT, SIPCA and Monthly Report to the MTL & Mission WatSan.
    • Ensure timely cleaning and maintenance of all water filter types, water storage tank/reservoirs, sanitation facilities and waste zone.
    • Ensure maintainance of all functional hand washing point in the health facility. Raising awareness amongst MSF beneficiaries and training personnel on the importance of hygiene and sanitation. This includes proper waste management in the hospitals, hand washing, food hygiene, safe water chain, warning on dangerous or toxic areas etc
    • The Watsan Supervisor ensures that all SOP and universal precautions are in place and followed. He/She identifies the needs for trainings and discusses those with the supervisor. In addition, she/he to supports the IPC supervisor to ensure that all IPC components are up to date in the health structures.
    • The Watsan Supervisor reports to the line manager on any concerns related to water supply, hygiene and sanitation, technical equipment, support structures and suggests solutions.
    • Training- He/she provide training for hospital cleaners and on-the-job support for hygiene agents.
    • Collaboration: He/she collaborates with the watsan supervisor of the emergency pillar during emergency reponse

    Requirements (Qualifications / Experience / Knowledge)
    Education:

    • Technical Diploma, desirable specialization in water and sanitation.

    Experiencel:

    • Minimum of 2 years experience.
    • Previous experience in technical works and organizing of multiple activities, preferably within MSF or similar work environment.

    Languages:

    Nurse (Referral)

    Background

    Main Objective and Responsibility of the Position

    • Providing nursing care, treatment and follow-up of patients, according to doctors’ prescriptions, protocols applied in service and universal hygiene standards/precautions, in order to ensure the quality and continuity of care for the targeted population.

    Accountabilities

    • Knowing, promoting, implementing and following up the universal hygiene standards/ precautions, bio-hazard prevention and infection control, security rules and other protocols and procedures in the medical premises and ensuring high standards of hygiene of his/her working environment. Respecting medical secrets and confidentiality at all times.
    • Organizing and carrying out care and treatments according to medical prescriptions, and assisting during consultations, daily rounds and other medical procedures. Participating in health education of the patient (and family) when necessary.
    • Identifying Sexually Gender Based Violence victims and referring them to the medical team so they can receive the necessary treatment. Knowing and is aware of the importance and appropriate use of the Post Exposure Prophylaxis (PEP) Kit
    • Ensuring patients are properly received and installed and that those with lack of autonomy are assisted, especially regarding their feeding, personal hygiene, movements and comfort. For ER and OPD nurse, ensuring triage of patients in waiting areas, wards or during emergencies, detecting the priority acute/emergency cases, carrying out first aid care and referring them to the doctor when necessary
    • Sending material to be sterilized and recuperates it from sterilization.
    • Carrying out admission, surveillance and follow-up of patients (i.e. assessment of the evolution of their state of health and the identification of any emergency situation or deterioration.
    • Carrying and supervising administrative procedures and documents (fill in patient’s files, forms, consumptions, statistics, registers, health files, etc.), ensuring an appropriate written/oral handover, and reporting any problematic situations and cases that may arise. Participating in data collection and keeping doctors/supervisors informed.
    • Participating in the department-related pharmacy and medical equipment control and maintenance (quality and its functioning, storage conditions, follow-up of expired drugs inventories, stock takes of medicines and material at every team changeover ensuring no material is taken out of nursing area/wards without prior authorisation, etc.)
    • Perform cleaning and minor maintenance for biomedical equipment used. Following the user manual and protocols and alert the supervisor in case of malfunctioning.
    • Supervising and training nurse-aids, nutritional assistants, cleaners, and other related staff under his/her supervision, ensuring teamwork and adherence to all appropriate protocols and procedures

    MSF Section/Context Specific Accountabilities:

    • To arrange the admission, and follow-up of the patients referred from ZSHCP Anka, Gummi ITFC, Sokoto ITFC, and other MSF sections to the validated referral centre (UDUTH) in a timely manner.
    • Keep record of patients needing further follow-up and ensure communication with such caretaker as a reminder in collaboration with the Health promotion team.
    • Ensure a proper guidance or orientation to the patients and caretakers on the hospital care, diagnostic tests and treatment plan that the hospital doctor suggests.
    • To be in the medical round with hospital doctors where MSF has a great number of patients and share feedback with line managers.
    • To ensure that diagnostic tests are conducted, treatments purchased, and problems encountered are communicated on time to Project Medical Referent in Sokoto/Zamfara project by using a database, which is supposed to be updated on weekly basis.
    • To serve as a link with the case between the referral centre (Gusau validated Hospitals, UDUTH), and the project/section that referred the patient as well as the PMR.
    • To ensure proactive communication the same day with the NAM/MTL in the case of challenges or problems faced at the Hospital level. In addition, if needed, to inform the project the same day as well.
    • The Referral Nurse maintains daily follow-up with each MSF-covered patient who is admitted in UDUTH Sokoto and reports on a daily basis to MSF MAM/NAM/MTL by phone or email.
    • The Referral Nurse will participate in analysing bills according to investigations and medications listed on each patient’s discharge sheet, and according to the hospital standard cost list. Any discrepancy will be reported to the management of the hospital for rectification.
    • To communicate with line managers in advance (at least 2 days) when MSF patients admitted is going to be discharged. Once the patient is discharged (or if death occurs), contact MSF MAM to make arrangements for the patient’s transport back to Anka or their villages. Cost for transport will be covered by MSF.
    • Complete Patient Discharge summary, patient tracking sheet, remaining advance, all receipts and submit the MSF MTL for approval. Provide patients with discharge summaries of their management and follow-up treatments if required.
    • To manage cash advance for transportation payment for discharged patients.
    • Supply drugs to the patients according to the prescription of the managing doctors in case medications prescribed are not available in the validated hospitals, in collaboration with the Hospital requesting that medication.
    • Report any problems that might be linked to the patients to the NAM/ PMR and respect the confidentiality of the patient history and medical file. In addition, ensure that the feedback is clear.
    • To be aware of any support needed from the patient regarding detailed responsibilities of referral nurse (ex. Changing prescription, procedures of getting Test, X-Ray, drugs, admission, discharge, death, self-discharge).
    • Ensure that Bills/Invoices from the Health facilities for referred patients are collected and submitted to the PMR with clear justification and relevant supporting documents/summary report without delay (every 5th of the month).
    • Follow up with the Health Facilities for any corrections and clarifications needed from the project regarding the verification process and address them accordingly. Ensure that rectified documents are forwarded timely.
    • Liaise with the hospital management in making sure blood is available for patients referred by MSF for blood transfusion in emergencies.
    • Maintain good working relationship with the management of the referral hospital and the medical team (doctors and nurses) that help to facilitate the care of patients referred by MSF.
    • Shall communicate with MSF MTL Sokoto for all matters regarding finance (advances) or anyone appointed by the MTL to undertake that responsibility while away.
    • Shall communicate with MSF MAM for all matters regarding patient care, patient outcome and treatments. In absence of MSF MAM, MSF MTL will be the next approved contact person.

    Requirements (Qualifications / Experience / Knowledge)
    Education:

    • Essential recognized Nurse Degree / Diploma.
    • Registered nurse RN, Accident, and emergency nurse, Desirable Bachelors in Science of Nursing.

    Experience:

    • Desirable 2 years of previous experience in that position and previous experience in other NGO’s

    Languages:

    • Local language essential. Mission language desirable

    Medical Doctor (IPD Paediatric)

    Main Objective and Responsibility of the Position

    • Provide outpatient and/or inpatient medical care to patients/beneficiaries according to adapted and updated medical knowledge, MSF protocols, values and universal hygiene standards to improve the patients/beneficiaries health conditions.

    Accountabilities

    • Apply medical knowledge and skills to diagnose and prevention. Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting MSF protocols.
    • Keep the patient and/or his/her family informed about the illness and provide appropriate explanations about the treatment to follow, checking they have understood.
    • Follow up the evolution of the hospitalized/IDP patients, through daily visits , consultations and examinations, prescribing the necessary treatment following MSF protocols, deciding whether they can be discharged or transferred to other departments – in collaboration with other doctors- and informing their family about the patient’s evolution.
    • Check and control the rational distribution of medicines and equipment under his/her responsibility and take care the quality, disinfection and sterilization of the medical material. Assure general compliance with standing hygiene standards.
    • Participate in the collection and analysis of epidemiological data, checking its validity and informing the line manager or project coordinator about any problem or complication of the patients’ illness, medical error and monitors the proper functioning of the department, equipment or material.
    • Ensure ongoing training of the medical/paramedical multidisciplinary team in order to optimize the quality of care.
    • Knows and ensures all MSF medical protocols are followed and implemented, checking universal precautions are followed at all times and reducing bio-hazard risks and improving infection control. Ensures professional confidentiality is respected.
    • Manage the team under his/her direct responsibility according to MSF HR policies and procedures, supervising their performance, organizing and scheduling shifts and rotations, directly participating in emergencies and on calls if necessary.

    MSF Section / Context Specific Accountabilities

    • Manage patients at an inpatient therapeutic feeding centre (ITFC) and children aged under-five with severe malaria and associated complications in a secondary health centre in collaboration with MoH MDs.
    • Apply medical knowledge and skills to diagnose and treat complications of common Paediatric conditions, Severe Acute Malnutrition and Moderate Acute Malnutrition. Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting MSF protocols.
    • Follow up the evolution of the Paediatric and ITFC patients, through daily visits, consultations and examinations, prescribing the necessary treatment following MSF protocols, and informing their family about the patient’s evolution.
    • Ensure rational prescription of medications and appropriate escalation or de-escalation of care depending on change in the patient’s condition.
    • Work on shift as per the roster in collaboration with MoH MDs.
    • Maintain and follow infection prevention and control practices and assure compliance of all the staff to the general hygiene standards. Ensure that universal precautions are followed and to organise a general standard of disinfection and sterilization.
    • Attend and conduct regular training for all medical and paramedical staff on Malnutrition, its complications and their management based on MSF protocols.
    • Ensure that data regarding a patient is collected and entered accurately and in a timely manner and, in collaboration with the Medical Data Processing Officer, ensure its completeness. To report any discrepancies to the Medical Activity Manager.
    • Uphold the dignity of the patient under their care and to practice in accordance with the principles of medical ethics.
    • Be willing to take up any role and support other members of the team as and when the need arises or if requested by the Medical Activity Manager.

    Requirements (Qualifications / Experience / Knowledge)
