Responsibilities: 

• Assess and monitor patients’ conditions.
• Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
• Conduct comprehensive health assessments and develop individualized care plans.
• Implement and evaluate patient care interventions, adjusting plans as needed.
• Educate patients and their families about health conditions, treatment options, and self-care strategies.
• Collaborate with physicians, other nurses, and interdisciplinary team members to ensure coordinated and comprehensive care.
• Administer medications, including IV infusions, according to established protocols and physician orders.
• Monitor patients for adverse reactions and report any concerns promptly.
• Maintain accurate and timely documentation of patient care, including assessments, interventions, and outcomes.
• Adhere to all nursing standards, ethical guidelines, and company policies.
• Participate in quality improvement initiatives and professional development activities.
• Provide compassionate and empathetic care to patients and their families.
• Advocate for patient rights and ensure their needs are met.

Qualifications:

• Bachelor\'s degree in Nursing or a related field. Nursing license or equivalent certification is highly desirable.
• Care Certificate is an advantage.
• Minimum of 3 years proven experience within the healthcare industry, preferably in a home healthcare setting.
• Strong knowledge of home healthcare principles, practices, and regulations.
• Excellent assessment, care planning, and case management skills.
• Ability to supervise and motivate Caregivers.
• Proficiency in Microsoft Office Suite and healthcare management systems.
• Excellent communication and interpersonal skills.
• Strong organizational and problem-solving abilities.
• Compassionate, empathetic, and client-centered approach.
• Ability to work independently and as part of a team.
