Interested and qualified candidates are required to submit five (5) copies of their Applications, along with a detailed Curriculum Vitae (CV) including the following details:

• Full Name, Date of Birth, Place of Birth, State and Local Government of Origin, Nationality
• Permanent Home Address, Current Postal / Contact Address / Phone number / email
• Marital Status, Number of children (with names and ages)
• Institution(s) attended (with dates), Academic and Professional Qualifications (with dates), Honours, Distinctions and Membership of Professional Bodies
• Working experience, present employment status, employer and salary
• Extra-curricular activities, as appropriate
• Names and addresses of three (3) Referees, two of whom must be professionals in the candidate's area of specialization
• Any other relevant information.
