5.) State of origin and Local Government Area
6.) Nationality
7.) Current Postal Address
8.) Educational Institutions attended with dates
9a.) Academic and professional qualifications and distinctions
b.)  Membership of professional bodies
10.) Work experience (in chronological order)
11.) Present employment status and salary
12.) Extra-curricular activities
13.) Publications (in standard format)
14.) Names and addresses of three (3) referees.
