Role Summary:

As a Medical Social Worker, you will play a vital role in providing psychosocial support and assistance to patients and their families to help them cope with the challenges of illness and navigate the healthcare system.

Responsibilities:

• Conduct psychosocial assessments of patients and their families to identify needs and concerns.
• Provide counseling and support to patients and families facing medical crises, chronic illnesses, or end-of-life care.
• Assist patients and families in accessing community resources and support services such as financial assistance, housing, and transportation.
• Advocate for patients' rights and coordinate with healthcare team members to ensure holistic care.
• Maintain accurate and confidential patient records and documentation.
• Collaborate with interdisciplinary teams to develop and implement care plans and discharge plans.

Requirements:

• Bachelor's or Master's degree in Social Work or related field.
• Valid license or certification as a Social Worker.
• Proven experience working as a Medical Social Worker in a healthcare setting.
• Strong knowledge of psychosocial assessment techniques and intervention strategies.
• Excellent communication and interpersonal skills.
• Empathy, compassion, and sensitivity to the needs of patients and their families.
