  • Names and addresses of three (3) Referees (Employer, Educational and Personal) who are to forward confidential reports directly to the Chairman, Gaya Polytechnic, Implementation Committee, with SUBJECT HEADING BEARING THE NAME OF THE APPLICANT, DEPARTMENT AND POST APPLIED FOR. Such referees should be able to attest to the Candidate's claim to high academic capabilities as well as to the Candidate's notable uprightness.
• Valid Government approved ID (Driver's License, National ID, Biometric Page of International Passport) and Medical Certificate of fitness from Government hospital.
• Two passport-sized photographs.
