Clinical Pharmacist at Sigma Consult

Role Summary:

As a Clinical Pharmacist at our client's healthcare facility, you will be responsible for providing direct patient care and medication management services. You will collaborate with healthcare providers to optimize medication therapy, monitor patient responses to medications, and ensure safe and effective medication use.

Responsibilities:

• Conduct medication reviews and assessments to identify drug-related problems and optimize therapy for individual patients.
• Collaborate with physicians, nurses, and other healthcare providers to develop and implement comprehensive medication management plans.
• Provide drug information and education to patients, caregivers, and healthcare professionals on medication use, adverse effects, and drug interactions.
• Monitor patients' medication therapy outcomes, laboratory values, and clinical parameters to assess efficacy and safety of drug therapy.
• Participate in interdisciplinary rounds, case conferences, and patient care meetings to contribute to patient care decisions and treatment plans.
• Document pharmaceutical care interventions, patient assessments, and outcomes accurately and timely in the electronic health record system.
• Participate in quality improvement initiatives and medication safety initiatives to enhance patient care and medication safety.

Requirements:

• Bachelor's or Doctor of Pharmacy degree from an accredited institution.
• Valid license to practice as a Pharmacist in Nigeria.
• Clinical pharmacy residency or equivalent training preferred.
• Proven experience working as a Clinical Pharmacist, preferably in a hospital or clinical setting.
• Strong clinical knowledge and understanding of pharmacotherapy principles.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a multidisciplinary team environment.
