    We are a health care organisation providing supported living and domiciliary care services to people in the community. We work across 2 local authorities and looking to expand our reach and services. We are looking for an energetic and forward thinking individual who is willing to join our team to achieve the set goals. As a Care Coordinator, you will be tasked with the responsibilities to support and promote a caring and supportive environment for Plan-It Homecare customers and staff by implementing very high standards of professional care and support.

    Key tasks for the ideal Candidate include:

    • Ensuring that all activities such as personal care and medication administration meet the required standards
    • Ensuring health and safety compliance
    • Planning and utilising resources and staying updated with the clinical knowledge to ensure that patients receive the best care
    • Ensuring that the staff deliver high-quality and personalised care to patients
    • Recruiting, training and supervising staff and communicating the needs of patients to Medical Staff
    • Complete the risk assessment and care plans for service users
    • Supervise the auditing team and the auditing of all documents relating to service users
    • Support in the training and development of staff around clinical matters

    Experience / Skills Required

    • Demonstrable experience of working as a nurse/ healthcare providers
    • Proven experience working with clients who have age related illness, learning disability and mental health condition
    • Excellent IT skills
    • Excellent customer service skills
    • Excellent problem-solving skills
    • Excellent communication and leadership skills
    • Positive attitude and keen to make a difference
    • Positive attitude to change and be innovative
    • Knowledge and understanding of abuse, safeguarding issues and ability to address them appropriately
    • Degree in Nursing or health related subject

    Salary: N250,000.00 - N300,000.00

    Schedule:

    • Monday to Friday
    • Weekend availability

    Experience:

    Management: 3 years (required)
