Each candidate shall submit Fifteen (15) copies of his/her written application with detailed
Curriculum Vitae (CV/Resume) duly signed and dated providing information as stated hereunder:

ii) Place and date ofbirth
iiii) Gender
iv) Local Government of Origin
v) State of Origin
vi) Marital Status
vii) Number of Children with Age
viii) Schools attended with Academic/Professional Qualifications
obtained with dates
ix) Previous Employment indicating-post and dates
x) Present Employment and postwith dates
xi) Administrative and Managerial positions held with dates
xii) Research and Publications with dates
xiii) Names and Addresses of Three (3) referees, one of whom should be the Chief Executive of his/her present or last place of work, and who should be in a position to attest to the candidates'
academic, professional and managerial abilities as well as moral character and uprightness.
xiv) Candidate shall also submit his/her Vision Statement for the Hospital in the next Four (4) years in no more than One Thousand (1000) words

Terms and Conditions of Service Applicable to the Post:
The remuneration and other conditions of service are as applicable to the post of Chief Medical Director in Nigerian Federal Tertiary Hospitals and as determined by the Federal Government and the Management Board of the Hospital.
